Every year, federally qualified health centers (FQHCs) and FQHC look-alikes are required to provide information about the health care services they deliver to underserved communities and vulnerable populations.
It is delivered via a standard UDS reporting system that collects consistent information year over year to comply with legislative and regulatory requirements, document program effectiveness, create benchmarks, and monitor trends.
The data collected includes patient characteristics, provided services, clinical processes, health outcomes, staffing, and financials. The report typically contains 11 tables and 3 forms.

Health Center Program awardees and look-alikes must report on a core data set and measures each calendar year as defined in the Uniform Data System (UDS), a standardized reporting system. HRSA uses UDS data to assess the Health Center Program’s impact and performance and promote data-driven quality improvement.
