Your 340B Rebate Model Pilot and MFP Checklist: How to Prepare for January 1, 2026

The January 1, 2026 launch of the 340B Rebate Model Pilot Program and the first round of the Maximum Fair Price (MFP) implementation is almost here. To help Covered Entities (CEs) proactively prepare for these major changes, Verity has created a practical checklist to guide your approach to new purchasing workflows. 

These tasks are meant to guide you on the best path forward for your organization at a strategic and operational level with Verity’s platform, V340B®. We also encourage current Verity customers to use the expertise of your Verity Account Manager while working through these steps before the January 1 launch. 


  1. Assess the Impact 
    Understand how the 340B Rebate Model Pilot and MFP drug purchasing will influence your existing workflows. Verity is providing tailored evaluations for all current customers to collaborate on a path forward using Verity’s platform and resources. 
  1. Communicate Early  
    Inform stakeholders about upcoming operational changes, including conversations around resources and financial workflows. Verity customers with questions around their own operational changes should connect with their Account Manager. 
  1. Register with Beacon 
    Sign up with Beacon Channel Management, the platform processing 340B rebate claims, to participate in the 340B Rebate Model Pilot Program. 
  1. Add Verity as Your TPA Partner 
    Enable claim submission automation with Verity by designating us as your preferred TPA on the Beacon platform. You will need the Verity access token to complete this process; current customers can contact their account manager for more details. 
  1. Strategize Medicare Part D Claims 
    Determine how you’ll approach Medicare Part D claims with inclusions or exclusions from your 340B program.  
  1. Upload the 340B Ceiling Price List to V340B 
    For Verity customers including Medicare Part D claims in their 340B programs, upload the 340B Ceiling Price List for MFP impacted drugs to the Verity platform to streamline purchasing decisions. 
  1. Define Accumulation Policies 
    Establish a clear approach for managing unreplenished accumulations and determine whether to apply manufacturer grace periods. 
  1. Review New Required Data Fields 
    Medical claims have additional required data fields you will need to include with rebate submissions. Verity has updated our file specifications to ingest these data fields, which customers can find in the Verity Help Center.

While Verity has been building new solutions for our customers compliant with the 340B Rebate Model and MFP requirements, we’ve compiled a list of the most frequently asked questions and their answers.  

How do I submit a 340B Rebate Model claim? 

  • All claims will be processed by Beacon Channel Management System, but your workflow is dependent on your current configurations and Third-Party Administrator (TPA). With Verity, we can enable automated claim submissions for eligible purchases to Beacon on your behalf—at no additional cost—to keep your program running as smoothly as possible. Manual submission is also an option; V340B can provide a customized Beacon export for CEs to conduct manual submissions.  

How are manufacturers providing 340B rebates? 

  • When a rebate claim submission is approved, rebates will be issued on a per claim basis—meaning the full package accumulation is not required. Per HRSA guidance, manufacturers are required to grant rebates within ten business days of submission. Verity has developed a deep integration with Beacon going beyond claim submission, which allows for data of automated submissions to flow from Beacon to V340B with rebate status details. Approved rebates will be provided directly to the facility.  
     

If I don’t participate in the 340B Rebate Model on January 1, will that impact my ability to participate in the future? 

  • No, there is no deadline to begin participating in the 340B Rebate Model once the program is live. For Verity customers who elect to participate after the expected January 1 launch, we will work with you to prepare your set up for your chosen start date. Until then, however, you will not be able to receive 340B discounts for drugs included in the Rebate Model Pilot. 
     

I need to order drugs included in the Rebate Model Program for Mixed Use and Owned Retail. Will I still be able to split orders with these specific drugs? 

  • Verity has updated our splitting logic in V340B to assess eligible usage and anticipate rebates based on accumulations. To submit a rebate claim for drugs included in the Rebate Model Program, the order must be placed on the 340B account. 

What happens if a manufacturer issued the MFP rebate, then later determines a claim should receive 340B reimbursement? 

  • In this instance the 340B rebate will also be paid, but the MFP rebate will be credited from the pharmacy by offsetting future dispenses.
